1. About Merlin™

Merlin™ Outcome Measurement Software runs on any standard personal computer. It includes BOTH version 1 and version 2 of the SF12™ and SF36™. For current studies, use version 1. For new studies, use version 2. Merlin™ comes complete with master questionnaires that you simply photocopy and hand out to clients. THERE ARE NO PER-QUESTIONNAIRE CHARGES! We also have available, as an option, the official Spanish translation of the SF36™ questionnaire.This software allows easy entry of SF12™ and SF36™ data. For instance, it only takes 30 seconds to enter an SF36™ questionnaire into Merlin™.Once data is entered, analysis is simply a matter of clicking on buttons. You can compare patients scores in multiple ways: compare pre- and post- scores on the same graph to see improvement over time; compare individual scores to national norms based upon gender, age group, and diagnosis; and compare individual scores to user-defined variables such as 'treatment' or 'therapist'.Merlin™ also has the ability to create user-defined statements on form printouts if patient scores fall below pre-determined levels. If, for instance, you are using the SF36™ in an orthopedic clinic a low "bodily pain" score might print out the message "consider referral to Pain Management Clinic".We can mail you additional material, such as sample forms if you wish. 2. Hooking up a scanner to your computer..

For large numbers of questionnaires, you can hook up a scanner to Merlin™! We can sell you the full package of Merlin™, scanner and Teleform Software (it reads data from your questionnaires into Merlin™). OR, if you have Teleform Software Version 6 and a scanner, we can sell you Merlin™ along with our scanner interface software. The Merlin™ scannable questionnaire consists of a one page SF12™, a two page SF36™ with a third page demographic that tracks diagnoses, procedures, treatments, ICD9 codes, and much, much more. 3. Creating a 'central processing point' for data collection via faxing from remote sites..

We can set up a central procession point for collecting patient assessment forms that are faxed from remote locations. We can set this up on our computer, or on yours. The central processing computer would scan the faxed form, score the form, and send the results in report or graphic output back to the sender within five minutes. This system could be set up to collect data from remote locations in different databases. Remote locations specializing in orthopedics would have their own database, and cardiac clinics would have theirs. The advantage of this is that over time you could create your own 'normative database' from these various specialties to compare an individual score to. This is a very powerful feature because you would create a level of predictability of complications, unnecessary hospitalizations, and other high-cost and high-risk outcomes.
